National Patient Safety Foundation’s (NPSF) Commitment To Privacy
Your privacy is important to NPSF. To better protect your privacy we provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used. To make this notice easy to find, we make it available on our Web site and where personally identifiable information may be requested.
The Information We Collect
This notice applies to all information collected or submitted on NPSF’s Web sites www.npsf.org, and affiliated member sites such as Stand Up for Patient Safety and Ambulatory Stand Up For Patient Safety.
Anonymous Information: When you visit any of NPSF’s Web sites to read or download information, we collect and store only the following information: the name of the domain from which you accessed the Internet; the date and time you accessed our Web site; and the Internet address of the Web site from which you linked directly to our Web site. None of this information can be used to personally identify you.
Personally Identifiable Information: The only personally identifiable information NPSF collects is information you provide to NPSF when you “fill out” one of the forms on our Web sites. You may use these forms to make contributions to NPSF, or to sign up for our e-Newsletter. The types of personal information NPSF collects vary by form and may include your name, post office address, e-mail address, phone number, credit/debit card information and answers to survey questions.
How We Use Information
- NPSF uses the anonymous information it collects to gauge Web site traffic, determine its most popular pages, and evaluate its Web site.
- NPSF does not share, distribute or sell e-mail addresses or other personally identifiable information.
- You can opt out of NPSF e-mail communications at any time. Each e-mail NPSF sends will indicate how to unsubscribe from the list via e-mail or via a Web page.
Third-Party Access to Personally Identifiable Information
NPSF provides personally identifiable information to its Web site service providers: PayPal, and the online store PSAWstore.com, on a strictly confidential basis to enable the platforms to maintain a payment database, send e-mail messages and provide other Web site-related services for NPSF.
Our Commitment To Data Security
To prevent unauthorized access, maintain data accuracy and ensure the correct use of information, NPSF has put in place appropriate physical, electronic and managerial procedures to safeguard and secure the information we collect online. We store your personally identifiable information on secure servers. Our third-party vendors accept credit card donations only via a secure server.
NPSF and “Cookies”
NPSF’s Web sites do not read, place or accept cookies.
Changes in Our Privacy Policy
Should our practices change, this notice will be immediately updated. Regardless of later updates, we will never use the information you submit under this current notice in a new way (in a way not described in this notice) without first providing you an opportunity to opt-out or otherwise prevent that use.
